I request to be admitted as a member :

of the Coopérative de solidarité du Suroît-CSUR which mission is:
- to exploit a business in order to provide work to its workers and a goods and services in :
a) Collective start-up initiatives
b) Telecommunications and any other similar activities

I declare that I am able to participate to the best of my competencies, interests and availabilities to the mission for which the Coop is constituted.

I commit to respect the rules, values and principles of the Coop.

In the case where I do not use the services of the cooperative as a user anymore, I ask the Coop to change my status to support member. In order to maintain an active status, I commit myself to notify the Coop of any change of address. If I do not keep my information up to date, I consent to give my social part to the cooperative one full year following my move.

I subscribe voluntarily 1 social part of 10$ for qualification to the Coop. This amount will be paid according to the modalities determined by the rules
I understand that I will become a co-owner, as an individual, of this social part and that it is my duty to ask for reimbursement or transfer to someone else with written notice to the administration council.
